Central Navigation is a voluntary, coordinated Community Response approach and single point of contact for children, youth and families when they need help and support. Central Navigation ensures that all children, youth and families have access to services and aims to connect them to resources and support in our community to increase youth and family protective factors. H3C Central Navigator is located within the Heartland United Way.
Central Navigator Information
Community Response Subcommittee Purpose
To lead and support the system of resources and services for children, youth, and families to strengthen families and build protective and promotive factors that can prevent families and young people from unnecessary entry or re-entry into higher-end systems of care.
Family Coaching/Case Management

Partner agencies meet with families to determine needs and develop a family sustainability goal plan. Referrals to this program are made by the Central Navigator.
